A doctor reads the blood-test clues.

“I’m always tired, and I don’t know why.”

Being always tired is one of the most common reasons people finally book a blood test — and one of the most frustrating, because “tired” points in a dozen directions at once. So let’s treat it like a case.

Eight hours of sleep and she still wakes up flat. Nothing dramatic — just a slow leak of energy that will not stop. Her doctor does the sensible thing: rather than guess, they line up the usual suspects and send off a panel of blood tests.

2The suspects

What usually drains your energy — and who we have to clear.

Both the NHS and Mayo Clinic point to this familiar line-up for unexplained tiredness. Our job is not to accuse the first suspect we see — it is to let the evidence clear the innocent ones and expose the guilty one.

3Gathering the evidence

Five clues come back. Four are cleared. One breaks the case.

Here is how a doctor reads the panel, one clue at a time — and you can check where each of your own numbers would sit using the free tools linked along the way.

Clue 1 — Thyroid (TSH) Normal

Her TSH sits comfortably in range, so an underactive thyroid is unlikely — a real result, since thyroid problems are a classic cause of fatigue. Check any TSH with the TSH interpreter or the TSH guide. Suspect one: cleared.

Clue 2 — Blood sugar (HbA1c) Normal

Her HbA1c — the three-month average of her blood sugar — is below the prediabetes threshold. Diabetes can cause tiredness, but not here. See how any HbA1c reads with the HbA1c interpreter or the blood sugar guide. Suspect two: cleared.

Clue 3 — Vitamin D On the low side

A little low — common, and worth topping up, but rarely the sole reason someone feels this wiped out. A person of interest, not the culprit. Check any result with the vitamin D interpreter or the vitamin D guide. We note it, and keep reading.

Clue 4 — The full blood count Quietly misleading

Here is where many people are wrongly reassured. Her haemoglobin — the headline number — sits at the low end of “normal.” A quick glance says “not anaemic, move on.” But look closer and her red cells are running slightly small, a subtle fingerprint that iron might be short even though the alarm hasn’t gone off yet. A good detective doesn’t stop at the headline.

Clue 5 — Ferritin: the smoking gun 8 ng/mL

Ferritin is the protein that stores iron — the single best snapshot of the body’s reserves. Mona’s comes back at 8 ng/mL, well below the level at which stores are considered low. This is the break in the case. Ferritin runs down before haemoglobin falls, which is exactly why she felt terrible while her blood count still looked passable — iron deficiency without anaemia, easy to miss if you only read the headline. Why the blood count looked “fine”

Iron stores empty out first. Anaemia is the last domino to fall — not the first.

Suddenly the whole story fits. Low iron stores explain the exhaustion, the breathlessness on the stairs, the thinning hair and the foggy afternoons — every symptom Mona described lines up with a single, quiet culprit a hurried glance would have walked straight past.

4Motive

Why the iron ran out

Finding low iron is only half the case — a doctor’s next question is always why. In Mona’s story the answer is ordinary: heavier-than-average periods losing a little iron every month, paired with a diet light on iron-rich foods — one of the most common patterns in women of her age. But “why” genuinely matters, because low iron can occasionally point to something that needs its own attention, which is why it is investigated rather than simply topped up and forgotten.

5The resolution

Case closed

With the cause understood, the fix is usually straightforward: replacing iron under a doctor’s guidance, supporting it with iron-rich food and a little vitamin C to help absorption, and — the step people forget — re-checking the ferritin a few months later to confirm the stores have actually refilled. Because iron rebuilds slowly, energy returns over weeks, not days. Her low vitamin D was topped up too. The tiredness that “came from nowhere” turned out to have a name all along.

6What this case teaches you

The point is never the patient — it is how you read your own results.
